Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about May 6, 2023, after reviewing the camera recording, it was discovered that AV open handed pushed Witness 1’s (W1) arms and W1 open handed pushed AV back. Between February 15th through May 6, 2023, AV had eight (8) incident reports written regarding physical behaviors. The facility failed to provide a safe environment for AV putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00008 $500.00 fine assessed
