Findings: Alleged Victim (AV) has a known history of falls. On or about May 11, 2023, AV had a fall and believes it was because he/she fell out of bed. As a result of this fall AV had a right orbital contusion, left elbow skin tear, left femur fracture and broken femur at the hip. AV had previously fallen on or about December 14 & 30th, 2022 and January 23, 2023. AV also had multiple falls between May 6th through 9th, 2023. AV’s service plan stated AV is not a fall risk. The facility failed to properly plan care around AV’s fall to further prevent any falls and or injury from occurring, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00049 $500.00 fine assessed
