- Findings
- According to the documentation, the facility failed to properly care plan for Witness 1’s (W1) behaviors and resident to resident altercations. The failure resulted in W1 wandering into the Alleged Victim’s (AV) room and pushing the AV down causing an injury, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
