Findings: Alleged Victim (AV) has a history of falls in h/h room when attempting to self-transfer to use the restroom. AV has interventions in place including staff providing locomotion to and from smoking area, keeping cigarettes at desk, providing routine checks before and after meals, placing an alarm on AV door, and reminders to use call pendant. On or about May 11, 2023, AV was found on the floor after attempting to self-transfer out of h/h bed and fell resulting in a fractured hip. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
