Findings: According to an investigation, on or about, May 5, 2023, Alleged Perpetrator 2 (AP2) took two checks from the Alleged Victim’s (AV) checkbook, forging and cashing one of the checks, in the amount of $400. AP2's actions are considered financial exploitation and constitutes abuse. The facility failed to keep AV safe from financial exploitation, which is a violation of Oregon Administrative Rules.
