Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV). The failure resulted in facility staff leaving the AV outside in the secured courtyard from approximately 9:00 pm to approximately 7:00 am without facility staff knowing they were outside causing prolonged unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
