Findings: The Alleged Victim (AV) relies on the facility for his/her care and has a history of falls. AV's service plan identified a high risk of falls with intervention of two staff assist with transfers and toileting. According to an investigation, between March 7, 2023, and May 18, 2023, AV experienced multiple falls when AV self-transferred. AV reported having to wait for extended periods when calling for assistance from staff, often having to wait more than an hour, resulting in unreasonable discomfort. A review of call logs verified extended call response times. The facility failed to answer call lights in a timely manner, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
