Findings: According to the documentation, the Unknown Alleged Perpetrator (AP2) failed to follow the Alleged Victim’s (AV) care plan during a transfer. The failure resulted in the AP2 pulling on the AV’s wrist to sit them up causing bruising on their wrist, which is a violation of resident rights and is considered physical abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative Rules.
