Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. The AV has a medical diagnosis which can cause AV to suffer from SOB and impact AV’s ability breathe. AV had approximately two separate occasions when AV asked for emergency medical services to be called so s/he could go to the hospital and was denied h/her request. AV was admitted to the hospital on the second incident with vitals significant for hypoxia, and AV oxygen saturation were low. The facility failure is a violation or resident rights, is considered neglect of care and constitutes abuse.
