- Findings
- The facility failed to implement interventions, appropriately care plan and supervise W1 and AV's according to their known behaviors. The failure resulted in a physical altercation where W1 struck AV in the arm causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
