Findings: Alleged Victim’s (AV’s) care plan states he/she is a fall risk; AV requires full assistance with transfers and is rarely or never able to communicate with or understand other. On or about May 21, 2023, AV had an unwitnessed fall with injuries; AV attempted to stand up and fell, hitting the side of his/her face on the bedside stand. As a result, AV sustained an abrasion to his/her cheek, both calves, and below his/her knee. AV was also injured in the right shoulder from the fall. On or about May 24, 2023, AV was found to have an abrasion/bruise from AV's right cheek area to under the right side of AV’s right eye; the abrasion/bruise was very red, with both bruising and scabbed area. The facility failed to provide a safe environment for AV, resulting in harm to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
