Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about March 15, 2023, AV had an assisted fall and continued to sway and stumble all night. On or about March 22, 2023, AV had a fall. On or about March 30, 2023, AV had an injury fall, staff found AV sitting on the floor outside of AV’s bathroom and AV was bleeding on the back of his/her head. AV was transported to the hospital where he received staples for the injury. AV is care planned to be a fall risk; however, no new interventions have been implemented to prevent future falls. The facility failed to care plan and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
