Findings: Alleged Victim (AV) has a known history of falls. On or about May 29, 2023, at around 7:20 PM, Av was heard yelling and crying, AV was found on the floor next to his/her bed. AV was sent to the ER on May 30, 2023, and returned with a fractured wrist. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
