Findings: The Alleged Victim (AV) is a known fall risk. AV suffered multiple falls, with and without injury. On or about March 22, 2023, AV experienced a fall and was not a baseline, AV was sent out to the hospital and returned March 26, 2023. On or about March 28, 2023, AV suffered another fall, resulting in a skin tear to his/her right elbow. The facility's failure to properly care plan to ensure AV's safety from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
