- Findings
- Alleged Victim (AV) relies on the facility to administer his/her medications. On or about May 5, 2023, UPA delivered a package of AV’s prescriptions to the facility and Alleged Perpetrator 2 (AP2) is who signed receipt and faxed as confirmation to AV’s pharmacy. AP2 stated that the package did not include a packing slip and accounted for a total of 30 tablets of opioid pain medication #1 and 90 tablets of opioid pain medication #2. AP2 is observed on May 5, 2023, on the facility surveillance footage opening AV’s UPS package and pulling out what appears to be a packing slip and proceeds to conceal the paper in a stack of other paper on top of the reception desk. AP2 is observed taking the UPS package out from under the reception desk and then walks out of the surveillance from frame. When AP2 come back to the reception desk in the view of the surveillance frame, he/she no longer has possession of the UPS package. On or about May 27, 2023, AP2 was interviewed by law enforcement in which he/she confessed to stealing one card of 30 opioid pain medication tablets, as well as one other card of 30 opioid pain medication tablets on May 5, 2023. AP2 failed to protect AV, from financial exploitation, which is neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
