Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about May 31, 2023, AV was heard yelling, when staff entered the room, they found AV laying on his/her roommate’s bed on his/her back. AV’s roommate was standing over AV holding him/her down by the wrists. Between February 15th through May 31st, 2023, it was listed 15 times in the Observation notes that AV had physical or verbal altercations or behaviors. AV’s care plan had no interventions in the Behavioral plan. The facility failed to properly plan care around AV’s known history of behaviors putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
