Findings: On or about May 31, 2023, two bruises were found on the inner thighs of the Alleged Victim (AV). No staff or AV knew how the bruises happened on AV. It was suggested that the bruises could have happened from staff rolling AV to change him/her, although it could not be determined which staff may have caused this. Alleged Perpetrator #2 (AP2, unknown) caused bruising to AV's thighs, which is a violation of resident rights, is considered neglect of care and constitutes physical abuse. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
