- Findings
- According to the documentation, the facility failed to implement fall interventions and to properly care plan for the Alleged Victim (AV) to prevent repeated falls. The failure resulted in the AV suffering a fall and sustaining bruising and swelling to their face and head, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
