Findings: On or about February 10, 2023, Alleged Perpetrator 2 (AP2), an employee of the facility, engaged in a physical altercation with Alleged Victim (AV) over AV's items being removed from his/her apartment . AV was a resident at the facility at time of incident. Based on interviews and facility documentation, AV suffered pain from the incident, requiring medication. AP2's actions is considered physical abuse. Alleged Perpetrator 3 (AP3) removed items from AV's apartment without AV's permission. AP3, an employee of the facility, wrongfully removed and disposed of items belonging to AV. AP3's actions is considered financial exploitation which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
