Findings: The facility failed to implement interventions and appropriately care plan related to The Alleged Victim’s (AV) known care needs. The facility failed to monitor AV in a manner necessary to maintain health and safety of AV and protect AV from being physically harmed by other residents. The failure resulted in two resident-to-resident altercations with separate residents in less than a week; one incident occurred on May 27, 2023, and the other incident occurred on June 1, 2023. AV sustained injuries during both incidents, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
