Findings: The Alleged Victim (AV) experienced an unwitnessed fall on or about June 2, 2023, no injuries were documented from this fall, however, it is documented that AV suffered pain from this fall and later had a bruise on his/her forehead. On or about June 3, 2023, AV suffered another unwitnessed fall, where AV was found between his/her bed and the wall. AV complained of neck, back and head pain. Emergency services were called and AV was transported to the hospital where he/she was diagnosed with a brain bleed and admitted to the hospital. The facility's failure to implement appropriate and effective interventions to protect AV from further falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
