Findings: The facility failed to provide a safe environment and to ensure that the Alleged Perpetrator 2 (AP2) was properly trained to secure residents during bus transport. According to an investigation, on or about June 6, 2023, AV’s wheelchair was not properly secured in the bus, causing AV to tip over. As a result, AV experienced back pain, dizziness, and a skin injury to his/her elbow. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that AP2 failed to a safe environment was investigated, and the determination was not substantiated.
