Findings: Alleged Victim (AV) relies on the facility for his/her care and is care planned to have their bed in the lowest position when not receiving care. On or about June 8, 2023, Alleged Perpetrator 2 (AP2) assisted the AV with care and did not move the bed back to the lowest position per the service plan. This resulted in AV falling out of bed and being injured. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
