Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) known resident to resident altercations with Witness 1 (W1). On or about June 11, 2023, Alleged Perpetrator 2 (AP2) heard screaming coming from the AV’s room. AP2 witnessed the AV leaving their room with W1 behind them hitting them and pushing the AV to the ground resulting in bruising and a laceration under the right eye. AP2’s failure to ensure the AV’s door was locked resulted in a resident-to-resident altercation which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility’s failure to properly care plan appropriate interventions to prevent resident to resident altercations is a violation of resident rights, is considered neglect of care and constitutes abuse.
