- The facility failed to follow Witness 1's (W1) care plan to redirect and provide appropriate supervisions according to his/her aggressive behavior and history of resident altercations. The failure resulted in W1 engaging in a physical altercation with the Alleged Victim causing unreasonable discomfort and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
