Findings: Alleged Victim (AV) is care planned to be a fall risk resident. On or about March 15, 2023, AV suffered a fall out of bed. On or about March 27, 2023, AV suffered a fall out of bed once again. AV then suffered an unwitnessed fall on or about June 4, 2023. On or about June 7, 2023, AV suffered a fall while attempting to undress and use the restroom, as a result, AV experienced knee pain following this fall. On or about June 19, 2023, AV suffered an unwitnessed fall and as a result AV sustained a head injury leading to bleeding coming from his/her head. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
