Findings: The Alleged Victim (AV) has a history of falls and is a known fall risk. On or about June 24, 2023, AV had a witnessed fall when getting up from his/her wheelchair and ambulating without assistance. AV requires staff assistance to walk or transfer to avoid falls. After AV's fall, he/she was noticed to have a swollen hand. AV was sent to the hospital and suffered and broken hand. The facility's failure to ensure proper supervision to ensure AV was safe from falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01015 $500.00 fine assessed
