Findings: The facility failed to implement appropriate interventions and monitor Alleged Victim (AV) according to his/her known suicidal ideation. The facility's failure to properly care plan according to AV's mental health care needs, resulted in AV attempting to kill himself/herself by suffocation with a plastic bag on June 3, 2023. AV attempted to harm himself/herself again on June 25, 2023, and again on June 30, 2023. The facility's failure caused AV unreasonable emotional discomfort which is a violation of resident rights, is considered neglect of care and constitutes abuse.
