Findings: According to the documentation, the facility failed to provide a safe environment to the Alleged Victim (AV) by intervening when Witness 1 (W1) was expressing increased behaviors. The failure resulted in W1 taking the AV’s walker and punched the AV in the face causing a laceration and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
