Findings: Alleged Victim (AV) is a high fall risk and is serviced planned for extensive assistance from staff with ambulation and mobility and requires extensive hands-on assistance from staff with transfers. On or about June 26, 2023, Alleged Perpetrator #2(AP2) was assisting AV to the restroom when AP2 left AV alone in h/h room to check on other residents. AV fell hitting h/h head, sustaining pain, swelling, bruising to head, neck, and back. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
