Findings: The Alleged Victim (AV) is care planned to be transferred with a gait belt. On or about June 29, 2023, AV was discovered with a bruise on his/her arm. Alleged Perpetrator #2 (AP2) admitted to not following the care plan by not using the gait belt to transfer AV. A few days later, AV was found with a bruise. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse.
