Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is a high fall risk with history of prior falls, and a broken clavicle prior to being admitted into facility. AV also uses a prosthesis. AV is care planned to use equipment or device, gait belt and prosthesis for pivot transfers. AV is a 2-person max assist for pivot transfers. AV also is to have a fall mat on the floor when AV is in bed, and make sure bed is lowered. On or about June 29, 2023, AV was found lying on the floor, bed completely elevated and no fall mat. On or about July 08, 2023, AV was found lying on the floor with no fall mat placing AV at risk for serious harm. The facility failed to provide a safe environment and provide appropriate services according to the AV's needs which is a violation of resident rights, is considered neglect of care and constitutes abuse.
