Findings: The Alleged Victim (AV) was incontinent of bowel and bladder, and as well was bed bound. On or about July 10, 2023, AV was found at the facility double briefed with an elevated temperature, altered mental status, and dry fecal matter. AV was transported to the hospital and the hospital could not rule out infection due to AV not being changed properly and double briefing is not acceptable practice, as it does not allow staff to know if AV needs to be changed or not. The facility's failure to provide proper incontinence care is a violation of resident rights, is considered neglect of care and constitutes abuse.
