Findings: The Alleged Victim (AV) is a know fall risk and has suffered multiple falls in February and March 2023. On or about April 22, 2023, AV suffered a fall while in his/her bathroom, suffering a skin tear to his/her hand, and pain to his/her head and left arm. Despite several falls during the evaluation period, AV's service plan remained unchanged when it was completed in March 2023. The facility's failure to properly care plan as it relates to AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
