Findings: The facility failed to provide appropriate services and care plan according to the Alleged Victim’s (AV) needs. Repeatedly AV has been found to be unkept and ungroomed, to be semi naked or in clothes that do not belong to AV, to have his/her room smell of urine and have bodily waste on the ground of AVs room, resulting in unreasonable discomfort and serious loss of personal dignity. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01379 $1500.00 fine assessed
