Findings: The Alleged Victim relies on the facility for his or her safety. On or about July 9, 2023, it was documented that AV had a bruise of unknown origin on his/her forehead. There is no documented evidence that the facility investigated where the bruise came from, ruled out abuse or neglect, or implemented additional interventions into AV's service plan to keep AV safe. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
