Findings: The Alleged Victim (AV) is a known fall risk and had suffered 5 falls within the last 30 days. On or about July 12, 2023, AV suffered a fall and was found on the floor near his/her recliner with a skin tear to his/her right eyebrow. While the facility care planned for AV's falls, the facility did not have enough staff to allow for one on one care, leaving AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01337 $375.00 fine assessed
