Findings: The Alleged Victim’s (AV) evaluation and service plan are dated June 19, 2023, almost one month prior to AV moving into the facility. AV moved into the facility on or about July 10, 2023. Prior to AV moving into the facility, but after the evaluation and service plan were completed, AV acquired a laceration to their hand which required ongoing treatment. The facility failed to update AV’s service plan and/or assess and treat AV’s hand and on July 14, 2023, AV went to the hospital for treatment. The facility failure caused AV unreasonable discomfort, is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-01310 $1500.00 fine assessed
