- Findings
- The facility failed to implement interventions and appropriately care plan related to Witness 1's (W1) known aggressive behavior and altercations. The failure resulted in an altercation with the Alleged Victim (AV) where W1 attempted to pull on AV where s/he had a recent amputation, causing risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
