Findings: Alleged Victim (AV) relies on the facility for a safe environment. AV service plan indicates AV has poor short-term memory, vision is diminished, has issues with h/h peripheral vision, does not wear glasses, needs assistance when sitting due to AV's vision, staff are to escort AV to meals and AV is independent in ambulation and transfers. AV is known to walk around the facility most of the day. The only intervention documented is to remind AV to use handrails while ambulating. On or about June 11, 2023, staff notice bruising of unknown origin to AV's left eye, left knee and right upper arm. Staff could provide no other explanation for AV's injuries other than that they were from AV bumping into things. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s visual impairment, and ambulating of the facility, which is violation of resident rights, is considered neglect of care and constitutes abuse.
