- Findings
- Alleged Victim (AV) relies on the facility for his/her care. Witness 2 (W2) and Witness 3 (W3) have a history of resident-to-resident altercations with AV. On or about July 15, 2023, AV was entering the facility when W2 and W3 blocked AV form entering, they begun to yell at AV and swore at AV, they also threatened AV and called AV names causing AV to feel afraid. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
