Findings: Alleged Victim (AV) relies on the facility for h/h care and for a safe environment. Prior facility history of similar occurrences provided passenger safety documentation. Passengers in wheelchairs should be removed from the chair and placed in a seat with seat belt on. On or about July 9, 2023, AP2 was reported as driving the facility bus erratically with AV in h/h wheelchair not properly strapped in. A walker not properly secured fell on AV resulting in a bruise and unreasonable discomfort to h/h left upper arm. AP2 did not follow passenger transportation safety guidelines, and a safe environment which is a violation or resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure a safe environment, which is a violation of Oregon Administrative Rules.
