Findings: On or about June 25, 2023 – June 27, 2023, Alleged Perpetrator 2 (AP2) signed they had administered Oxycodone to Alleged Victim (AV) on MAR however no medication was logged on AV's Oxycodone administration sheet. AV had a shortage of Oxycodone at the end of June. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
