Findings: Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has a known history of sexualized behaviors towards AV and (opposite gender) residents and staff. On or about June 18, 2023, while seated next to AV in a common area, W1 reached over to grab and rub AV’s inner thigh and appeared to tap AV’s genital area with their hand. AV does not appear to have capacity to understand or communicate his/her needs, which makes AV quite vulnerable. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
