Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) known history of exit seeking. On or about July 26, 2023, the AV eloped from the facility for an undetermined amount of time. At approximately 6:47 pm, the AV was seen trying to enter the facility through the front door. Upon evaluation, the AV sustained an abrasion to their elbow, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
