Findings: The Alleged Victim (AV) moved into the facility on or about July 03, 2023. AV had approximately five (5) falls from about July 06, 2023, to July 30, 2023, resulting in a head injury, AV becoming unresponsive for a brief episode and transported to the hospital, bruising to right shoulder, vertebrae and buttocks, skin tear to the left wrist and pain. The facility failed to provide a safe environment, appropriately care plan, and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01517 $500.00 fine assessed
