Findings: Alleged Victim (AV) relies on the facility for a safe environment. Witness 1 (W1) has had a prior Resident-to-Resident altercation. Staff know they must always be in the dining room when residents are present. On or about August 28, 2023, AV crossed the dining room and began unlocking the wheelchair of another resident at W1's table. W1 kicked AV, AV then hit W1 twice striking h/h in the face. The Facility failed to provide adequate supervision and a safe environment during mealtime for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00041 $500.00 fine assessed
