Findings: Documentation provided to the department shows Alleged Victim (AV) is scheduled to receive showers (bed baths or sponge baths) twice per week and if AV refuses both showers in a week to notify family member to come in for assistance. Staff to ensure Av’s feet are dry between toes after shower and before assisting to dress. AV received approximately five showers in the month of July 2023. AV received a shower on or about July 19, 2023, complained of feet pain on July 20th and per documentation received h/h next shower on or about July 27, 2023. AV left fourth (4th) toe was noted to have a wound. AV was sent to the hospital on or about July 30, 2023, with an infection that required a surgical amputation of h/h left fourth toe. The facility failed to provide appropriate services according to the AV’s needs, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
