- The facility failed to follow the Alleged Victim's (AV) care plan related to fall interventions, which included frequent safety checks due to AV's impulse to self transfer. The failure resulted in AV experiencing an unwitnessed fall causing head pain and a hip fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
