Findings: According to the documentation, the facility failed to provide transfer assistance for the Alleged Victim (AV) when they wanted to get up. AV is a 2-person assist with Hoyer lift for transferring. The AV requested to be transferred but there were not available staff to assist at the time of the request so they could attend activities. The AV experienced discomfort and was not able to participate in preferred activities due to staff not transferring the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
